A suggested donation of $150.00 per student is requested, with an additional $25.00 for those enrolling in music or dance classes.

Waiver and Release of Liability – Shree Pashupatinath Foundation, USA – Nepali Pathshala
In consideration of my child/children being allowed to participate in Nepali Pathshala classes and related activities organized by Shree Pashupatinath Foundation, USA, I, the undersigned parent or legal guardian, hereby acknowledge and agree to the following:

Assumption of Risk: I understand that participation in Nepali Pathshala activities, including physical movement, cultural programs, and other class events, carries a risk of injury. I voluntarily assume full responsibility for any potential injuries or medical conditions that may arise during participation. I accept financial and legal responsibility for any such incidents.

Release and Indemnification: I hereby release, discharge, and hold harmless the Nepali Pathshala Steering Committee, Teachers, Shree Pashupatinath Foundation, USA, its Board Members, volunteers, and any facilities used for the program, from any claims, liabilities, or damages resulting from injury, loss, or disability related to participation in the program.

Medical Responsibility: I understand that Nepali Pathshala, its committee, board members, and volunteers are not liable for any accidents, injuries, or medical/dental expenses incurred as a result of my child’s participation.

Health Insurance and Fitness: I confirm that my child is in good health, physically able to participate in class activities, and is covered under our personal/family health insurance.
I HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE OF LIABILITY. I VOLUNTARILY AGREE TO ITS TERMS WITHOUT COERCION. I AFFIRM THAT I AM THE PARENT OR LEGAL GUARDIAN OF THE CHILD/CHILDREN NAMED ABOVE.
